If you are a new patient to our office, please fill out the relevant forms below when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your dental needs more quickly than completing them on your arrival. Staff members are available during these hours to answer your questions, make appointments, and take messages.
If you have an emergency, please call us as early in the day as possible and we will do our best to respond to your dental emergency promptly.
We participate with most of the insurances.You will need to supply us with the necessary information (name, group number, I.D. and/or social security number, telephone number, etc.). Insurance companies do not cover all dental costs. It will be your responsibility to pay any deductible or any other non-covered services. We do not accept secondary insurance.If you have secondary insurance, you submit your own claims.We will give you all the needed information.But still you have to pay us according to your primary insurance plan.
We accept following methods of payment:
- Cash, Debit or Credit Card.
- 5% cash courtesy for advance payment of entire treatment plan.
- We accept MasterCard, Visa, Discover and American Express.
- 3, 6, 12, or 18 month – No interest monthly payment plans as well as low interest and extended payment plans through Care Credit.
We are happy to estimate fees and insurance reimbursement before starting recommended treatments and will gladly file insurance forms for you. Optimal, comprehensive dentistry is what we practice and we do not base our treatment on what insurance will or will not pay. We base our treatment on what is best for you and any treatment not paid by insurance is your financial responsibility.
Doctor’s and Hygienist’s time is reserved only for you. If you need to make any changes to your appointment, please give us 2 days of notice in advance or a charge of $50 will occour to your account. These changes will make us utilize our time efficiently for the smooth running of the office and also will let us allow the needed time for those patients who need their dental emergencies to be taken care of.
